Difficult Discussions Sessions

From Our School Nurse

It is that time of year to re-issue MAFs (Medication Administration Forms) to students who received medications and treatments in school. As you may remember these forms must be completed every year.

Since we can not issue from the school, I will be contacting parents/guardians informing them of this information.

The forms are now available on line: infohub.nyced.org/in-our-schools/translations/forms/health-forms-and-notices

Please use the link to find the appropriate form needed

In addition please note the return addresses:

New York City Department of Health & Mental Hygiene

Office of School Health

Gotham Center

42-09 28th Street, CN-25
